2= I live in an urban area with more than 20 000 inhabitants.
3= I live in an urban area with more than 200 000 inhabitants.
4= I live in a big city with more than 1 million inhabitants.

How often do you find yourself in crowded areas?
1= I never find myself in crowded areas.
2= I try to avoid crowds by taking my car or my bike, but sometimes I do find myself in crowded areas.
3= I’m in crowded areas a few times during the week.
4= I’m in crowded areas everyday (bus, subway, etc).

How often are you exposed to cold weather?
1= Never; it’s sunny every day of the year where I live.
2= Sometimes the weather is a bit grey and cold, but most of the time it’s beautiful outdoors.
3= Half the year it’s nice; the rest of the year it’s cold, cloudy or rainy.
4= Everyday here feels like winter.

How much exercise do you get?
1= I try to exercise daily
2= I try to do a physical activity once or twice a week.
3= I’m trying to move my body from time to time.

Between work, studies and family life, it’s sometimes difficult managing a schedule. How would you describe your daily routine?
1= My day passes very calmly. I get up whenever I want and do just what I feel.
2= My daily routine is busy, but not that bad. I still find time for all the things I like to do.
3= I have a stressful daily routine, but I still find some time for myself.
4= I have an overloaded schedule. I rush so often that I barely have time to breathe.

How would you describe the level of stress in your life?
1= Nothing causes me stress or anxiety; I’m always pretty relaxed.
2= I have a medium level of stress – we live in a nice country, after all.
3= I have some periods when my level of stress is very high.
4= I’m tense and nervous almost everyday.

How healthy and balanced is your diet?
1= My diet includes fruits and vegetables. I eat regularly, and only healthy foods.
2= I’m trying to balance my diet with fruits and vegetables, but sometimes I also eat some things I shouldn’t.
3= I’m aware of the principles of healthy nutrition, but I usually find it hard to follow them.
4= I don’t pay any attention to what I eat. I like junk food and sometimes I don’t eat at all.

How much sleep do you get?
1= I always get the sleep I need and I usually sleep very well.
2= I sleep well about half the time.
3= I go to bed very late and always feel I miss some hours of sleep.
4= I’ve forgotten what it means to sleep well at night.
